Care at home is an important resource to support our seniors to maintain their dignity and quality of life in the very comfort of their homes. Our care models are individualized to meet each patients’ specific needs. Being able to live at home offers a sense of safety, comfort and independence and our goal is to ensure that our clients experience this. All one may need is some assistance with personal care, activities of daily living and meal preparation. Our certified home health aides provide all these services with great compassion in the comfort of your home. At Maybridge, we do not require long term contracts or minimum hours. Companion Care at home
Provide companionship and assistance for seniors in the comfort of their home to keep them safe and engaged. Studies have demonstrated that those aging adults stay healthier with social interaction. Home health aides can become trusted friends for walks, reading, cards, games, movies, meals, and other social activities. Accompaniment on errands such as grocery shopping, medical appointments, and other activities also benefits home care services.
As part of this service, we provide assistance with activated of daily living, meal preparation and light housekeeping. Our team members are all certified home health aides who are compassionate and caring.
After Hospitalization Care at home
As we grow older, we may have complex needs after hospitalization or having surgery. Being able to gain one’s independence and stay safe through the recovery process may require some help for a trained home health aide. This is where Maybridge can be of assistance to you. Home Care prevents an increased rate of hospitalization and readmissions and also allows patients to be discharged home in a timely manner as opposed to waiting for a rehabilitation bed.
Post-operative Care
One main focus after surgery is ensuring that transitioning back to your normal routine progresses safely and successfully in order prevent injury or falls. Areas such as dietary intake, activity of daily living and medication management are some of the areas we can assist in your transition. We will be present to ensure that your discharge plan from your surgeon is followed so you are safe and recover well.
Dementia and Alzheimer’s Care at home
We provide a wide range of services such as companion service which helps with activities of daily living and recreational activities, home maker services which included light housekeeping, meal prep and shopping and care coordination with skilled care providers.
Cancer Care at home
During or after cancer treatment, recovery in the comfort of your home makes life much easier. We provided a wide array of services that assist our patients recovering from cancer focus solely on their recovery. Our home health aides provide assistance with activities of daily living, light housekeeping, meal preparation and companionship. All these services are provided with certified home health aides who have additional training in Cancer Care at home.
Respite Care
We know caring for a loved one can be very rewarding and also a daunting, as such we provide assistance by providing support to alleviate care giver fatigue. Our services are flexible, and we customize them to meet your individual needs.
Skilled Nursing Care Services
Our skilled nursing care includes direct care such as catheter changes, observation and assessment of your condition, management and evaluation of your care plan, wound care, certain injections, tube feedings, and teaching about prescription drugs or diabetes care.
Private Duty Nursing
Private duty nursing provides patients with long-term skilled nursing care in their own home. Services include but are not limited to assisting patients manage their health, be it a chronic condition or not. Private duty nursing provides a long-term solution which is intended to improve a patient’s well-being over time. This service is a one-on-one in-home service provided by a licensed and certified nursing professional.
